Student E

Student E is a student with complex medical health needs that just finished requirements for 4 years with their co-hort and the IEP team determined the need for Beyond 18 programming to help support the student, family and agencies as they transition to Day Services support and explore options in the community.  Due to complex medical health needs it was determined there needed to be some overlap of services so staff could support each other in this transition and support relationships built between student, mom and care givers.

Team Members: Student, Mother, Long Term Support Case Manager, Day Services coordinator and job coach, Special Education Teacher, School Nurse, Student’s Home Health Nurse, ArtStart Director, DVR, Director, Occupation Therapist, Physical Therapist, Speech and General Education Teacher.
