Volunteer/Aide Intake Form
Please take a moment to download and complete this fillable PDF intake form. The "Certification" section refers specifically to any teaching certificates that you may have earned. The "Years of Experience in Adult Education" refers to any time you have previously spent teaching adults and when you started. You can leave the "Start Date for This Agency" blank for now; we will fill that in later.
Copy of Bachelor's Degree (or higher) - 1:1 Tutors Only
We are required to keep a copy of your bachelor's degree if you are a one-on-one tutor. Classroom aides are exempt from this requirement. Copies of transcripts indicating which degree was earned are also acceptable and are frequently the cheapest solution.
Please contact your original degree-granting institution if you need to order these documents.
