Waiver

Align Training & Pilates (ATP) Client Waiver of Liability & Release

In consideration of my participation and/or that of my child or ward in Align Training & Pilates and it's staff and affiliates (henceforth known as ATP) programs or activities, do hereby for all, to the extent permitted by law assume the risk of participation and waive and release ATP from any and all claims, actions, or demands for any injury, loss, or damage arising out of, or related to participation in the programs or activities, whether or not such is caused by the act, error, omission, negligence or fault of ATP. I also on behalf of myself and my child or ward consent to ATP's publication of photographs taken of any of us during our participation in ATP's activities for use on ATP’s websites and social media. 

Terms & Conditions:

• I agree to adhere to all ATP’s policies and procedures:

• I certify that I have answered all health and fitness questions honestly and to the best of my ability.

• I understand that for my safety, I may be asked to provide a medical clearance from my doctor prior to participating in any personal training.

• Full payment is due prior to services being received and payment can be accepted via Cash, Check, Venmo, or Credit Card. 

• If I need to cancel my session, I must call, text or email my trainer at least 24 hours prior to my scheduled session. If I do not call 24 hours prior, that session will be forfeited without refund.

• If I am late, the session will only last until the end of the allotted time which that session was scheduled. If I am more than 15 minutes late the scheduled session will be forfeited.

• If my health status changes after beginning training, I will inform my trainer immediately. I understand that I may need to obtain a physician’s clearance prior to continuing training sessions.

• All sessions must be used within 90 days of the original purchase date.

• All sales are final; No refunds on sessions or packages.
