Cane Creek:

• Co-visit clinic model with pharmacist and provider for chronic disease state management (diabetes, HTN, anticoagulation) and Medicare annual wellness visits

• Location – 1542 Cane Creek Rd, Fletcher, NC 28732. Meet Olivia in shared kitchen/office workspace.

Diabetes QM/DPP:

• Quality measure chart reviews for patients with diabetes with Andy Warren and pre-diabetes screening service as a part of the Diabetes Prevention Program (DPP) with Anna Pfeiffer

• Location – 123 Hendersonville Rd, 2nd floor student work room or remotely off campus

Dispensing:

• Learn about 340b pharmacy practice, medication access and medication dispensing. 

• Location - 121 Hendersonville Rd, door on front right of building

Endo Pharmacy clinic:

• Pharmacist run clinic for management of type 1 and type 2 diabetes management, CGM and pump trainings.

• Location - 123 Hendersonville Rd, basement floor (clinic rooms vary). Meet in IM precepting room to debrief patients. 

FM clinic:

• Pharmacist run clinic for chronic disease state management (diabetes, HTN, anticoagulation) and Medicare annual wellness visits

• Location – 123 Hendersonville Rd, 1st floor (usually clinic rooms 14-15). Meet in resident office to debrief patients.

FM consulting:

• Review messages in FM pharmacy clinic + student inboxes. Answer questions from medical residents or faculty in Family Medicine

• Location – 123 Hendersonville Rd, 1st floor. Meet in 1st floor precepting room. 

IM didactics:

• Weekly lecture style series to understand prevention, diagnosis and treatment of adult related diseases

• Location – 123 Hendersonville Rd, SIM CENTER in basement of OB/GYN building.

IM Kubiak & Mueller covisits:

• Co-visit clinic model with pharmacist and provider for chronic disease state management (diabetes, HTN, anticoagulation) and Medicare annual wellness visits

• Location – 123 Hendersonville Rd, basement floor (clinic rooms vary). Meet in IM precepting room to debrief patients.

IM pharmacy clinic:

• Pharmacist run clinic for chronic disease state management (diabetes, HTN, anticoagulation) and Medicare annual wellness visits

• Location – 123 Hendersonville Rd, basement floor (clinic room 1). Meet in IM precepting room to debrief patients.

IM resident clinic:

• Co-visit clinic model with pharmacist and resident for chronic disease state management (diabetes, HTN, anticoagulation) and Medicare annual wellness visits

• Location – 123 Hendersonville Rd, basement floor (clinic rooms vary). Meet in IM precepting room to debrief patients.

Med Access:

• Referral based service to help patient’s afford their medications through local, state and national resources

• Location: 123 Hendersonville Rd, 2nd floor Family Medicine building across from printing room.

OB/GYN:

• Co-visit clinic model with pharmacist and provider for gestational diabetes, hormone replacement and contraception.

• Location – 119 Hendersonville Rd, 2nd floor Green Daisy station. Meet at nurses Green Daisy station.

Osteoporosis Clinic:

• Co-visit clinic model with pharmacist and provider for osteoporosis and osteopenia management.

• Location – 123 Hendersonville Rd, alternates between Family and Internal Medicine clinics.

Project Time:

• Designated time to prepare for projects and topic discussions

• Location – 123 Hendersonville Rd, 2nd floor student work room or remotely off campus

PCCM:

• Primary care clinical meeting – lunch and learn session with local pharmacists 

• Location – 123 Hendersonville Rd, Health Sciences Large Classroom or virtual via Webex if off campus
