The ALS Association Connecticut Chapter (ALSA CT) collaborates with Coordinated Transportation Solutions (CTS) of Trumbull, CT– a full service vendor that administers the ALSA CT Transportation Program for people living with ALS in CT. This program is dependent on available funding.
Eligibility Criteria
• Person must be registered with ALSA CT and live within CT.
• Person must not have any other alternative transportation services available (such as transportation available through Medicaid or the VA).
• Person must be in contact with their ALSA CT Regional Care Manager to receive the online transportation application form. The application must be completed and returned before transportation can be scheduled.

Service Availability
The transportation program accommodates both non-wheelchair and wheelchair users. The program may be used for round-trip rides to and from ALS Clinics and/or Neurology offices in CT. The program may also be used for special events such as weddings, other medical appointments, children's' events, etc. Special requests for above this criterion may be considered by the Executive Director. Rides are approved based on available funding.
How to Make a Request for Round-trip Transportation
Once you have completed the application and receive confirmation from your Regional Care Manager that you are enrolled in the ALSA CT Transportation Program, it is the responsibility of the person living with ALS or his/her caregiver to contact CTS to schedule transportation at least 7 days in advance.

Please be prepared to provide CTS with the following information: Location, Date, Time & Duration of your Appointment.
