Suwa Surakum Welfare Fund Facilities
Members of the Suwa Surakum Welfare Fund of the Sri Lanka Air Force Ex-Servicemen' Association can obtain the following facilities/ benefits.
1. In the case of a death of a member a death donation of Rs.70,000/= will be paid to his/her spouse or to the next of kin subsequent to a formal request.
2. In case of a death of the spouse, a death donation of Rs.35,000/= will be paid to the member subsequent to a formal request.
N.B.If a member of the Sri Lanka Air Force Ex- Servicemen's Association has not obtained the Suwa Surakum Welfare Fund membership, the member's spouse or the next of kin will receive a death donation of only Rs. 10,000/= from the Sri Lanka Air Force Ex- Servicemen's Association
3. If the children of a member enter a State University, until their university studies are over a sum of Rs 700/= per month scholarship grant will be paid subsequent to a formal request.
4. Suwa Surakum Welfare Fund Guidelines for Medical Grant Approvals.
4:1A medical grant will only be considered for conditions that need hospitalization and treatment (for surgical and medical cases)
a. Major surgeries such as CABG, Stenting, Heart (Up to Rs.50,000/=)
b. Major surgeries such as Cancer, Liver, Kidneys, Lings, Bowels, Nephrectomy, Neuro surgeries, permanent injuries (Up to Rs.40,000/=)
c. Minor surgeries - under local Anesthesia (Up to Rs.30,000/=)
d. Any other case, if necessary, will be considered on case-by-case basis (Will be decided by the Board of Trustees on case by case)
e. Medical treatments needing hospitalizations will be considered on case-by-case basis on seniority and will be decided by the Board of Trustees on case by case.
f. Expenses on special investigations e.g.: Angiography, MRI Scanning, Dialysis (up to Rs.20,000/=)
g. IOI - Cataract (Rs.6,000/=)
4:2: Drug bills and the claims requested for LAB investigations will not be considered unless they are hospitalized during that period. Drug bills will be considered for payments only during the hospitalized period when those drugs are not available in the hospital.
4:3: However, there is no provision to consider payments for medical bills.
4:4: Repeated drug bill payments - Patients who need continuous treatments shall claim these drugs from nearest SLAF medical Centers.
4:5: Decision of the Board of Trustees is final in all matters.
4:6: All claims shall be submitted within 3 months from the occurrence.
4:7: Members will be eligible to claim medical grants only after 3 months since the date they obtain their SSWF members
