When you choose to contact us by the above methods to initiate contact with the service a record of this contact will be kept whilst correspondence/providing of services is still active. The information provided by you will be retained in its original form. This information may be stored within our email system, or on mobile phones. This may include your name, contact number and email address. By getting in contact with us, the user consents to be contacted in response to their query.
If you do not respond to contact, your details will not be retained. If you inform us that you do not wish to continue contact, your personal details will be deleted.
A client’s personal information will be stored for the purposes of providing services. This may include name, phone number, address, email address and other personal details. The information will be stored securely, for example in password protected documents on password protected devices. Or it may be stored within email or phone where these accounts and devices are secured with passwords. Any information gathered via paper form will either be stored securely in a locked facility or be destroyed appropriately once converted to an electronic record.
Records created due to accessing therapy will be kept either electronically or by paper. Paper records will usually not have personally identifiable information on them and will be kept in a locked cabinet.
In order for you to access the service, we need to store information about you to arrange appointments. This information may include your name, email address, and phone number, and any other information you have provided to us. This information will be used solely to provide the service.
Information retained for completing services:
If services are accessed, records of this will be kept for invoicing purposes, as well as for medical and legal reasons. Records will be stored securely and kept for the legally required period of time.
We will not disclose or share your information to third parties. Should you be referred to this service via a third party, for instance a health insurer, communication will take place between this service and the referrer, who will have their own policy relating to retaining your personal information.
As therapists are required to have clinical supervision to maintain quality standards and ethical service, clients may be discussed in an anonymous way with the clinical supervisor. All clinical supervisors working with this service adhere to the same ethical standards of practice and confidentiality. As a precaution I have a Clinical Will, which includes a password protected list of clients to be contacted in the event of an emergency where I am unable to contact them myself. This list is only accessible by myself, however it will be sent to my clinical supervisor in an emergency situation so that he is able to contact my clients to advise them of my unavailability. Until then my supervisor only holds the document password.
Confidentiality will be maintained unless during the sessions you suggest that there is a risk of immediate harm to yourself or to others. Should this issue arise therapists have a legal duty of care to share this information with the appropriate authority and can be done without consent, although, where possible, the therapist will discuss this with you first. Such services may include the emergency services, your GP or other healthcare professionals, or social services.
