The system should have emailed them an appointment reminder. Please have them use the link within the email to complete their medical history prior to coming for the appointment. If they are unable to fill out the electronic health history – please have them fill out the attached form.
We see patients within Malcolm Grow Medical Center. Second floor to your left, if coming up the stairs, Oral Surgery Suite. (Directly above the pharmacy)
