CVS Aenta/Caremark - BC Monthly Reverification - use following spreadsheet to complete orders each month. CVS Team Lead will notify those approved to work this sheet when it is ready.

For MOC statuses that you obtain from ABMS that you would choose an MOC status that contains "not listed" for either Time Limited certs and/or for Lifetime certs that say that they are not meeting/participating in MOC please check the Boards regular website to see if it gives more direction there. For example Lifetime certs for American Board of Pediatrics on ABMS will say not participating or not listed but on the American Board of Pediatrics site it states that MOC is not required. We need to return what the Board site says in these situations.
If there is no Expiration date listed after the - and issue date you will leave the expiration date blank, we will not use the "reverification date" as the expiration date in these cases.
When processing in ABMS and the Status for the Certification is "Certified" and if "Pending Certification updates from Board" is indicated and/or the listed Exp date is in the past you need to go to the Boards website and obtain the correct Exp date from there.
Work through orders top to bottom
Select Medpass link provided in Column H. View provider medpass.
Find All Specialty Boards Pending Results order listed under verifications (this should always be in pending status, reach out to lead if it is not)
Open Order > Select Eagle Gate Order
Pull up BC Processing Rules for Board Name listed. Follow rules for specific board. (Usually utilizes ABMS Certifacts Login Required for this site)
Fill out all Fields for Eagle Gate Order
First Name, Middle Name, Last Name - Copy what is listed on board site
Specialty - Copy what's listed on Board Site Rules/Board
Board Name & Authority Source - Copy from Board Site tab
Eligible status - use appropriate status listed on Board
MOC - Yes/No- Found on the Board Cert
Lifetime - Yes/No - Found on the Board Cert
Lifetime Reason No: - see MOC rules below
Accrediting Agency - Copy for Board Site Rules Tab
Board Certification Number - if given on BC copy over to Eaglegate
Fill out dates appropriately based on rules
Notes- Use general clipping
Verified status: reflect what BC states
Upload PDF.
Copy URL and then Submit order.
Paste Eagle Gate URL in column J.
Fill out G, I, K and L
Move to next order.
These are generally used for All Professions besides Osteopathic (DO) Providers:

When you will want to use it: When the Board states the Provider is not certified.
When you will want to use it: When the Board Certification was completed as Not Found
When you will want to use it: Board Certification status shows Inactive
When you will want to use it: Verification shows Pending with a Issue date shown.
When you will want to use it: Verification is a Lifetime certification and states that they must meet MOC and are not currently meeting MOC.
When you will want to use it: Verification is a Lifetime certification and states that they are meeting MOC
While the ABA strongly encourages all diplomates to participate in MOCA, participation is voluntary for diplomates certified prior to 2000.
Most diplomates certified prior to 1990 are not required to participate in MOC but are strongly encouraged to do so.
All ABIM certifications issued in 1990 (1987 for critical care medicine and 1988 for geriatric medicine) and thereafter must be maintained through ABIM's MOC program.
Not Required for Lifetime Certifications.
MOC is also available to time-unlimited (lifetime) certificate holders who wish to voluntarily participate in the MOC process
An Active Diplomate with a time-limited certificate is subject to all Certification Maintenance requirements.
