Instructions:

1) Please download and fill in the HIRARC form in the first link below.

2) Fill in the form in 3 copies and obtain your supervisor's verification on the form.

3) After completing, please scan and upload the soft copy of the form in the given link (go to sheets for FYP1/FYP2 respectively).

4) You also need to 

• Keep one copy with you when entering the lab.

• Submit one copy to the ikohza. 

• Submit one copy to your supervisor.
