How to use the template

Make a copy using a Google account

• Google provides a free forms tool that will make collecting responses easy. If you don't already have one, sign up for an account. 

• Log in to your account and then make a copy of the form using this link.

Customize the form template for your needs

• Add your water system name in the title of the form

• Based on the capacity of your system, you may want to remove the question about offering an in-person inspection or free water sampling.

Sending the survey URL

• At the top of the form edit page, click the big "Send" button

• Then choose the "Link" option in the window that appears

• Copy this link URL

• Use this link to direct customers to the survey.
  ➡️ Put a link to the survey on your website, and tell people to go there since your website likely has a shorter and easier URL. 

How to view responses

• Responses can be viewed under the “Responses” tab at the top of the form edit page

• You can also create a Google Sheet to view responses in spreadsheet format, which can be helpful for sorting and tracking.
