READINESS VENUE
You can sign up for one of the readiness venues for an HIV draw, vision screening or immunizations (Please annotate what immunization is needed on the sign up sheet if you know.).
Students: If you are outside of the NCR and need a medical readiness requirement, you will need to transfer your care to your current location and visit the nearest medical readiness clinic to fulfill your requirements.

Please complete the online portion at your branch site, listed below. Once completed, send an email to usumedicalreadiness@usuhs.edu stating you have completed the online portion. Please include your DODID. Once we have confirmation that your online portion has been completed and we have your DOD we will contact you if any further action or information is needed. If you do not hear back from me within 5 business days please do not hesitate to reply back to the chain for an update.

IMR UPDATE
Please email usumedicalreadiness@usuhs.edu and provide us with the information that needs to be updated, include your DODID. Please know, we will NOT respond letting you know it is completed. For the Army, please allow up to 1 week for it to show up in your IMR. For Navy and Air Force, Please give us up to 5 business days to complete. We will contact you if we have any questions or need more information.
MILITARY PHYSICALS
For all physicals requiring a 2807 and 2808, part 1 needs to be completed at the WR readiness clinic.
We cannot schedule your part 2 until part 1 is completed. Once part 1 is completed, send an email to usumedicalreadiness@usuhs.edu with the following information:
-The type of physical needed (ex. Retirement, Commissioning, Airborne physical)
-Provider request (If you have one)
-Your DOD
-The time frame you are looking to book your appointment
-Good contact number
