AESP RENEWAL

1. 1. Complete two forms (health history questionnaire and risk exposure assessment form) linked below. Once completed, you may provide it to UOHS either via email employeehealthquestions@usuhs.edu or in person (A1037). Please indicate on the attached form if your risk category has changed in the past year (refer to attached form).

USU to USU email is encrypted, but outside email addresses are not. If you wish to send your health history with a password, you must save the document as a PDF format, then go to "Properties" and enter your password. The password should be sent in a separate email.*

1. 2. UOHS will follow up via email to provide your AESP card or request more information if needed.

1. 3. You will receive your AESP card as a PDF through your work email.

1. 4. Please email the AESP card (along with the DLAR Access Request Form) to dlar-admin-ggg@usuhs.edu
