Students must be current on all state required immunizations to attend school. Or All students must be current on all state required immunizations. For legal alternatives see K.S.A.72-6262 and contact the school nurse prior school attendance.
All parts must be completed by parent and provider to be considered complete. Please double check this prior to turning them in.
If your child has received any new required vaccinations, a copy should be given to the school nurse to update students health file.
Health assessments are required by the State of Kansas for any student ages 0-9 years old enrolling in school for the first time or any student newly enrolling to a Kansas school.
