Who is my Registration Adviser?

You can check your adviser here.

My current adviser will be on study leave. What should I do?

Inquire with the S-A-P Division Head about your new adviser. After knowing your proposed adviser, accomplish DSS Form 02 - Request for a Change in the Plan of Study.

I will be taking my first 3-unit of SOC 200 next semester. What section should I enroll in?

Be sure you have submitted your concept proposal during the Call for Submission of Concept Proposal that happens at the end of the semester or midyear.
After submitting the proposal, you will be given an Undergraduate Thesis Adviser. Every adviser has their own SOC 200 section. So, you must confirm with your Thesis Adviser the section you need to enlist. Also, please be reminded of the section code. The first three letters correspond to the Faculty-In-Charge. The last letter corresponds to the unit you should take. (A-1 unit; B-2 units; C-3 units; D-6 units).

How do I request a permit to transfer/shift?

Accomplish OUR Form 2021 - 6 if you are applying for shifting to another Degree Program Within the College (Shifting 1) or to another Degree Program from Constituent Universities (Shifting 2).
Write a request letter to apply for shifting/transfer addressed to the College Secretary, 'noted by' your parent or guardian, with the 'recommending approval' of your adviser, S-A-P Division Head, and Department Chair. This application letter will serve as one of the attachments to your application form.
Your adviser will ask you to answer the exit survey before signing your letter, so do not forget to inform your adviser after you have accomplished the survey form.
Upon receipt of the signed documents, you may submit them to the CAS-OCS.

When do I apply for my Elective Plan of Study?

You may accomplish your Elective Plan of Study (DSS Form 01. Elective Plan of Study) after completing your foundation courses (SOC 10, PSY 10, POSC 10, ANTH 10, and HIST 10). Based on the curriculum, you are supposed to complete these foundation courses during your first year in the program.

When do I apply for my GE Elective Plan of Study?

What should I do if I need to change my elective courses?

Consult with your adviser and accomplish DSS Form 02. Request for Change in the Plan of Study.

I am a transferee/shiftee. How would I know if my courses taken at my previous university/college can be credited?

Consult with your adviser regarding the courses you have taken at your previous university/college to check if these courses can be credited. After evaluating the courses, you will be asked to accomplish a letter indicating the courses to be credited. (Please attach a copy of the syllabus). Please note that this only applies to students who shifted from other colleges and transferred from other UP constituent universities.

I only have 12 units. Does that mean I need to apply for an underloading permit?

Yes, you need to apply for an underloading permit. To justify underloading, the submission of pertinent documents to the Office of the UniversityRegistrar through the Office of the College Secretary is required as follows:1. for health reasons—medical certification to be confirmed by the University Health Service;2. for unavailability of subjects—certification by the adviser and copy of schedule of classes;3. for employment—copy of payroll and appointment papers indicating, among others, the duration of employment. It is the responsibility of the student to establish beyond reasonable doubt the veracity of the cause(s) of underloading. It is required that documents submitted to establish the cause(s) of the light loading must be sworn to4. for no more courses to take based on the curriculum - certification by the adviser that there are no more courses to take.

I was able to register 18 units plus HK and NSTP this semester, do I need to request an overload permit from the OCS?

No, HK and NSTP units are not counted as academic load.

Do I still need to apply for my elective plan of study if I already took the courses?

Yes. Since you took the course before submitting your elective plan of study, you need to write a letter to the Department Chair endorsed by your adviser explaining the negligence committed before you can submit your elective plan of study.

I need to drop a course this semester without a PASS or FAIL evaluation, what should I do?

Write a request letter addressed to the CAS-OCS indicating the subject/s you are planning to drop and the reasons for dropping 'noted by' your parent or guardian, with the 'recommending approval' of your adviser, S-A-P Division Head, and Department Chair.
Accomplish the DROPPING Form to be issued by the CAS-OCS upon the approval of your request.

How to process college clearance?

Check with your adviser if you have deficiencies in the department (exit survey for shiftees/transferees, borrowed books from the DSS Reading Room, thesis manuscripts, etc.) before accomplishing CAS College Clearance Form.

I want to know about other courses offered by the Department (e.g., prerequisites, semester offerings, etc.). Where can I find them?

I am planning to request for reinstatement to UPLB and continue my degree in BA Sociology from AWOL (Absence without Leave of Absence) status. What should I do?

Write a request letter addressed to the Vice Chancellor for Academic Affairs indicating the semester you plan to be reinstated, the reasons for being AWOL, and the plan of action. Check with the CAS-OCS the supporting documents you need to prepare (e.g., affidavit of support, evaluation sheet, True Copy of Grades, Plan of Study or list of all courses taken and to be enrolled in the upcoming semesters, plan of action, etc.).

I have already exceeded the maximum rule for residency (MRR). What should I do?

You need to request for extension of residency. Write a request letter addressed to the Vice Chancellor for Academic Affairs indicating your request for extension of residency and your plan to finish your remaining subjects. Check with the CAS-OCS the supporting documents you need to prepare (e.g., Plan of Study or list of all courses taken and to be enrolled in the upcoming semesters).

What is the difference between a request for reinstatement and a request (appeal) for readmission?

A student requests for reinstatement if the student did not enroll for the past semester(s) without applying for Leave of Absence (LOA), while a request for readmission is for a student who appeals to continue studying in the university despite incurring a scholastic status of dismissed or permanently disqualified.
