Local Coordinating Coucil (LCC)

What is LCC?

Scheduling: LCC will use a universal schedule to maintain consistent and predictable meeting times throughout the Faribault and Martin Counties. Same school scheduled on the same day of the same week of the month.

Once a referral is received, they will be placed on the schedule in the next available time slot. Every effort will be taken to schedule around working parents/guardians schedules.

Each meeting, consisting of the student, parents/guardians, human service worker, court service worker, law enforcement, school personnel and other appropriate supports, will last for thirty minutes. During that time, strengths of the student and family will be discussed as well as areas of concern. The meeting will be wrapped up with an 'action plan' for the student/family, and/or another member of the team to accomplish by the next scheduled meeting time.

A copy of the notes from the meeting with the action plan will be sent home with the family. The date and time of the next LCC will be noted on this plan.

The LCC process strives to improve and support the lives of children and families who have complex needs.

Appropriate referral: A student who has multiple needs (school, home, court services, human services, community).

The family must sign a release of information. This allows the LCC access to work and talk with the family in developing the best overall plan for the family.

The emphasis of the LCC efforts are to identify these multiple need situations and bring together the family and all appropriate service providers in order to create one service effort that supports families living more successfully.

Families must be at the LCC meetings. Parents-guardians are encourages to be active members. The LCC is an opportunity for students and parents/guardians to feel supported, heard and praised for their actions. Emphasis is placed on a team approach and using strengths to help make better decisions.
