8th Grade Update from Nurse McCaw
To Parents of 8th Grade Students:
The Illinois State Board of Education requires that all 9th grade students have an up to date school physical and immunization record on file with their school. Unit 5 has a first day exclusion policy, which means that your student's physical and immunization record needs to be turned into the school nurse prior to the first day of 9th grade. Please do not wait until the last minute to schedule your student's appointment because doctor's offices get extremely busy at that time.
If you recently turned in a physical for your student, please check with your school nurse to see if the 9th grade physical and immunization requirements have been met.
(Please note- if an IESA/IHSA sports physical was the only physical turned in, this does not meet the requirements for a 9th grade school physical. It must be a State of Illinois Certificate of Child Health Examination form).
A dental exam is also required in 9th grade but that does not have to be turned in until April 2025.

YEARBOOKS!
Yearbooks will be distributed on May 17th for those who purchased books ahead of time. Teachers will be going through the list of names of students who purchased a book and that list will also be hung in the pods. Students should be double checking that their name is on the list if a parent ordered for them.
Please note: Every year there is always one or two students who did order a book but their name doesn’t populate in the list from Jostens. IF that happens, please email the receipt of the purchase (Jostens emails receipts right after purchase) to Mrs. Poulos so we can make sure your student gets their book. Her email is poulosab@unit5.org
If you didn’t purchase a book for your student and want to, we will have a limited number of books available for purchase starting Monday, May 20 in the front lobby outside of the office. It will be a first come, first serve basis. We cannot take any money ahead of time to reserve those extra books. The price is $35 cash or check made out to Evans Junior High. Thank you!
