This requirement is non-negotiable for all crew (new and returning). It is a condition of working on a USCG-regulated vessel. This must be completed within a week of receiving your offer letter. If your start date is less than a week away, complete immediately. 

Anderson-Kelly is a third party administrator that determines medical fitness for crew. You must be medically fit to work on the vessels for safety and compliance. If you have questions please contact HR@uncruise.com.

Instructions

1. You must use a computer or tablet to complete the medical forms online.  If you are not able to access a computer or tablet, click here to download the paper version. 

2. The password to open the online forms is Uncruise2017

3. After completing the medical forms, you will receive an email confirmation titled "You signed: "UCA_Medical Forms_2017_Rev1". Forward this email confirmation to HR@uncruise.com and keep this email for your records.

4. Once you have read 1-3 above, click here to access online medical forms through Anderson-Kelly. Follow the prompts as directed. Please note the following:

• • The forms must be completely filled out to submit. 

  • Form B2 asks for "date of exam" information. You are not required to participate in a medical examination to complete this form. If you have not had a recent medical examination, please write "N/A" in that area.

Notes about Anderson Kelly

Needs Further Review (Medical) Status = This means that AK needs additional information before completing their medical assessment. They will follow up with you directly on required documents. It is your responsibility to provide the requested documents prior to arriving to the vessel. If you are having communication issues with AK, please email HR@uncruise.com.
