Follow the below steps to complete the medical requirement via paper version:

1. Instructions on how to fill out the medical questionnaires

2. FORM A – HIPPA Authorization: Print legibly, sign and date

   • Email a photo to HR@uncruise.com and scheduling@andersonkelly.com OR fax to (973) 446-2615 (Anderson Kelly)

3. FORM B – Medical Examination History: Print legibly, sign, and date

   • Email a photo to scheduling@andersonkelly.com OR fax to (973) 446-2615

4. FORM C – Consent Form - DOT Regulation 49 CFR 40.25: (complete ALL areas of the form, i.e. phone #s, dates of employment, and your signature)

   • Email a photo to scheduling@andersonkelly.com OR fax to (973) 446-2579 "Attention SAPP"

5. Click here to notify UnCruise Adventures that you have completed your medical form.
