Once the Attestation Form and Quiz Orientation have been completed please scan all pages into ONE document and send to Heather Adams via email (hadams@udel.edu) in the following format:
Last Name, First Name- St. Francis Onboarding *(documentation not submitted in required format will be returned to student(s) to correct and re-submit)
* St. Francis requries a few addtional screenings/clearances that must be run by their agency. Monica Richards, (monica-richards@che-east.org) from St. Francis Human Resources, will be reaching out to students individually via email for the required signatures/documenation needed.
Updated Drug Screening Required
Students assigned to St. Francis are required to have an updated drug screening within 60 days of their clinical start date. Please see Castle Branch order code & addtional details below:
FAILURE TO COMPLY WITH REQUIRED ONBOARDING DUE DATES WILL RESULT IN THE FOLLOWING:
Students will not be permitted to enter clinical site, nor participate in clinical until requirements are met
Possible removal from course
