A member of the Clinical Coordination Team will schedule an orientation (with Taryn Pariag, Bayada Clinical Educator) for students to meet and review important information prior to the start date of clinical.
Please review the following forms prior to scheduled orientation:
Student Honesty & Confidentiality Training
Staying Healthy " A Guide to Infection Prevention "
Taryn will have students sign stating they read and understood the information at orientation
Per clinical site, all students are required to be N95 Fit Tested prior to the start date of clinical. A member of the Clinical Coordination Team will provide the required due date and necessary instructions. Please see directions below:
Combine the completed Respirator Fit Test Form & Health Screening Provider Opinion Letter into ONE document (PDF) and email to Heather Adams (hadams@udel.edu) in the following required format:
