Willow Brooke Court at Cokesbury Village
As required by the clinical site, Cokesbury mandates that all students be N95 fit tested prior to the start date of clinical. Please review and follow the important instructions below. Due dates will be provided by a member of the Clinical Team.
Step 1:
Please complete the OSHA Respirator Medical Evaluation Questionnaire
(Part A, Section 1, Questions 1-12 & Part A, Section 2, Questions 1-15)
Step 2:
Please send a copy via email of your completed OSHA Respirator Medical Evaluation Questionnaire to our undergraduate coordinator, Jen Eldred (jshearer@udel.edu) in the following format:
Last Name, First Name-N95 Questionnaire
Step 3:
Approval Process: Once your OSHA Respirator Medical Evaluation Questionnaire has been reviewed by Jen, she will reach out to each student individually and provide either clearance to sign up for a fit test appointment or instructions for any additional steps that may be required.
Step 4:
Once you have received clearance, a Sign-Up Genius link will be provided by Heather Adams for students to begin signing up for their N95 fit test appointment.
Please Note: students may not schedule a time slot until they receive the above-mentioned clearance from Jen
Upon receiving certification, Jen, Abby, and myself have been granted permission to complete fit testing appointments right here at McDowell Hall in room 223-this makes things so much easier 😊
