Required Student Onboarding 

Per clinical site, all students are required to be N95 Fit Tested prior to the start date of clinical.  A member of the Clinical Coordination Team will provide the required due date.  Please see directions below:

1st step: 

Call Nurse Managed Primary Care Center (NMPCC) to schedule a pyhsical/evaluation appointment & Email Heather Adams (hadams@udel.edu) your scheduled appointment date and time

(A member of the Clinical Coordination Team will provide the NMPCC in advance with a list of student's names. They will try their best to work everyone in around class schedules.) 


3rd Step:  

Email Heather Adams (hadams@udel.edu) your completed Respirator Fit Test Form (as a PDF) in the following format:

Last Name, First Name-N95(Rotation 1 or Rotation 2) 

*Documentation that is NOT submitted in the following format WILL be sent back to student(s) to correct and resend 

2nd step: 


Once you have completed your physcial/evaluation appointment @ the NMPCC, call the Environmental Health & Safety Office (EHS) to schedule your N95 Fit Test appointment & Email Heather Adams (hadams@udel.edu) your appointment date and time
