Testimonial Forms

After our Fall Quarter Town Hall, Oakes affiliates raised ongoing concerns about campus lighting, CruzFix maintenance response times, and mold in residential spaces.

To address these issues and advocate for lasting improvements, the Oakes Student Senate has created the following testimonial forms. These forms allow students to share their experiences and specific problem areas directly with us.

All submissions will be compiled into a comprehensive report that will be shared with our Senior Director and Facilities teams to ensure that student voices and lived experiences directly inform future repairs and renovations at Oakes College.

• All forms are anonymous by default unless you choose to include your name and email.

• The Mold & CruzFix Training Form is intended only for CruzFix workers to share experiences regarding their training, equipment, and safety conditions.
