Electives

ORGANIZING YOUR ELECTIVE

Please note residents must organize electives directly. Please get in contact with the appropriate admin to organize.

FOR LOCAL ELECTIVES

At least one month in advance, complete the Local Elective Request Form which requires the following information:

• Name of Elective

• Dates of Elective

• Destination of Elective

• Your DOB; PGY level

• Preceptor’s Name and e-mail address

FOR OUT-OF-TOWN ELECTIVES

At least three months in advance, (4 for international electives), complete the  Occupational Medicine Elective Form  which requires the following information:

• Name of Elective

• Dates of Elective

• Your DOB; PGY level

• Destination, including complete mailing address

• Contact name with e-mail address

• Your home address

Any elective forms requiring signature by Dr. Durand-Moreau are to be forwarded to mepocmed@ualberta.ca
