Volunteer Registration / screening requirements:
We need all these forms submitted from you in order for us to provide you with references or reference letters.
For 2022/23, please upload all your forms here.
IF UNDER 18: Please fill out the informed consent form
Have a reference fill out this form for you. A reference can be anyone that can comment on your personal characteristics such as a previous music teacher, supervisor, coach, etc. Returning volunteers do not have to fill out a reference form.
If you have an existing police check that was done within the last 3 years, you can just take a picture and upload it.
If you don't have an existing police check, you can get one at any local police station. Bring the personalized police check letter attached the email you should have received to get it done for free. Unfortunately, this cannot be done online as there is no way to account for the personalized letter requirement – EPS is currently working on this.
EPS has currently paused their police checks for youth, so if you are a minor who does not currently have a police check, please reach out to us as soon as possible so we can coordinate supervision.

Please pay attention to these forms especially because they are a requirement given to us by the University for our volunteers / students to fill. Original copies must be returned to us (no email or scan). Both forms must be printed double sided ; but you may print them in color or black & white.

Name of participant is the child's name ; not the parent's name
Anyone (i.e. you as the volunteer) can be the witness as long as they are not a family member of the child ; ensure the witness writes their address as well in the designated spot
