Nurse End of Year Reminders
Parents/Guardians must pick up any medications, inhalers or Epi-pens by Thursday, May 25, 2023 in the nurse’s clinic. Remaining medication will be discarded at the end of the school year, if not picked up per TPS policy.
Vaccine News: The new school year will be here before you know it! Make sure that your child has the vaccinations required by Ohio law. New Kindergarteners need their shots updated and upcoming 7th Graders require a Tdap booster and a meningitis vaccine.
TPS School Based Health Centers offer vaccination: Rams Health Center (419) 442-7702, Bulldogs Health Center (419) 442-7701, Waite Health Center (567)204-3056
Also, additional options for vaccines, call your child’s doctor or visit a Shots 4 Tots immunization clinic, call 419-213-4121 or visit http://www.lucascountyhealth.com for information about Shots 4 Tots and Teens. Please provide a copy of shots to your school nurse. Children will be excluded from school for not having their shots up-to-date or on file at school.
Schedule your daughter’s doctor appointments for vaccines and medications! Medication forms can be completed at your daughter’s appointment. Medication forms must be completed each school year, this includes: prescription, inhalers and Epi-Pens. In general remember that all children should get a yearly well check and eye exam. This will help assure that your child is on track and up to date on health needs and vaccines. Remember the longer you wait the longer the lines!
School nurses do not have summer hours. Any medical changes or medical forms can be returned at the start of the school year, faxed to (419)671-5395, or mailed to the school:
