Please take this form with you to the doctor if your child needs Prescription (RX) or over-the-counter (OTC) medication administration at school. Each medication will needs its own form (2 medications=2 forms). Students are required to have this form completed by a physician for any medication dispensed at school.

This form is ONLY for students in GRADES 7-8. This gives the GUARDIAN the ability to give consent for TPS to administer Tylenol and/or Motrin to your child for minor pain/conditions not related to head injury. Due to TPS policy change, NO DOCTOR SIGNATURE is required for these two over-the-counter (OTC) medications to be administered at school.
