The following questionnaire will be provided in the event of a POSITIVE COVID-19 TEST. Parents will need to provide this information to the school IMMEDIATELY or to the Lucas County Health Department. If time does not allow the school to gather the information at the time of the call, the school will conduct a follow-up call as soon as possible. Please have the information to this questionnaire readily available and any contact numbers that might be relevant. Thank you.
