The Clinical Quality & Site Complaint form is for anyone in the Company who receives a client complaint, or escalation, that involves (1) Clinical Quality, and/or (2) Office Environment.

A clinical quality complaint is one made regarding the:
Professionalism
Reliability
Ethics
When there is a complaint made against a clinician regarding one of these categories, a Clinical Quality and Site Complaint Ticket must be completed.

An office environment complaint occurs when there is an accessibility issue within one of Thriveworks' physical locations.
The complaint will be reviewed and investigated by representatives of Clinical Operations, Compliance and Credentialing. Legal and Human Resources may be brought in to assist in the investigation as warranted. The investigation includes: discussing the issue with the complainant, discussing the issue with subject of the complaint, and any witnesses as required. Documentation also may be reviewed.
The outcome of a complaint may result in board reports or termination, only as needed.
You may check in with your supervisor on the status of your complaint.
