A clinical incident is any of the following:
A client providing information on a planned or intended death, including suicide, and any report made;
Any high risk behavior of a client, including a threat or attempt of harm or suicide, that puts a client or others at risk for safety, and any report made;
Assault, homicide or other crime resulting in client death or major permanent loss of function, and any report made;
Sexual or any other perceived inappropriate behavior displayed by client;
Assault or threat of violence made by client against clinician or others;
Death of a current or former Thriveworks client;
Knowledge or reporting of child, disabled persons, or elder abuse;
Suspected ethical violations by a clinician, including fraud, waste and abuse;.
Police involvement or arrest occurring at a Thriveworks location;
Disturbance, threats or acts of violence at a Thriveworks location;
Property damage to a Thriveworks location;
Injury to a Thriveworks employee or client occurring at a Thriveworks location;
Any other incident in your reasonable judgment that should be reported.
If an incident has occurred the first step is for you to reach out to your RCD to discuss as soon as possible. It is important that these issues are handled promptly, as time is of the essence and can help us avoid any further issue or escalation. After speaking with your RCD you will be directed to fill out our Clinical Incident and Injury Form. When completing this form, please be as detailed as possible, as this will help our Incident Review Team appropriately classify the incident and will determine appropriate next steps to manage the issue.
Once the form has been completed, our Incident Response Team will determine appropriate next steps, which often includes meeting with your RCD to discuss appropriateness of care, other clinical implications, and whether there are any legal or compliance issues. At that point, your RCD will reach out to you to discuss case management and clinical next steps, closing out the incident.
