Any letters provided by Thriveworks clinicians should only state the following:
Dates of treatment
Diagnosis
Attendance
Treatment Plan Goals, progress, and objective outcomes on symptom reduction
Letters should never include:
Opinions
Theories
Recommendations on legal matters, discussed in other sections of this document, particularly custody
Expert testimony or expert opinions
Fitness on someone’s readiness or fitness to return to work or school
Letters that are permitted:
treatment summaries
verification of client status
verification of dates of treatment
Letters you should avoid writing:
letters on fitness to return to work or school
custody recommendations
opinions on an individual's need for/mental capacity to consent to surgeries

You are able to charge for this letter, which importantly, will be self-pay for the client. We do encourage you to discuss this with the client prior to providing the service. After a letter/form is completed, you would complete a misc/contact note to document and bill. Please see the steps to do so below:
1. On the EHR side, open the patient's chart, by typing in their last name, first name within the search field.
2. You will now be in the client's EHR chart. Under the Notes card, click the green plus to add a new note. This will open a blank note for you.
3. On the info page, you will need to make 2 selections. First, you would want to select the template type of "Unlinked notes-Use Dashboard to Charge." You can then select the template to Contact note/Misc. Note.
4. You will now have a "Note" tab at the top of the note. You can complete that tab in its entirety and sign. Also, encourage you that select the proper billing code at the bottom of this tab.
5. After signing the note, a charge slip will generate. Please sign that, after reviewing.
Below are the costs for the client are listed below:
Client Forms (Physician Statements, FMLA, etc.)
Level 1 Paperwork (0-20 mins)-$50
Level 2 Paperwork (21-45 mins)-$100
Level 3 Paperwork (45-60 mins)-$130
*Except disability form charges in states where prohibited: GA, NJ
