NOTE: You must use your personal Thriveworks email account. Shared ones will not work.
visit the FreshService Home Screen
Select “Request a Service” on the Home Screen of the Fresh Service Portal.
Select AMD/ EHR, then choose the option that best fits your need
You can keep track of existing tickets by looking at the bottom of the screen from the Home page in the Portal where it shows:
Open tickets
or you can select the link in emails.
**Any AMD user activation, termination, rehire or role change must be submitted as a request by the user's direct supervisor.**
