Pre-Program Requirements

Please submit the following to Summer Springboard NO LATER than the due date listed.

• Due at time of Application

  • Enrollment Essay Questions

  • SSB Terms & Conditions Form

  • Medical Questionnaire

• The required forms listed below are now available in the Parent/Student Dashboard. The deadline to complete each form is listed in the dashboard, along with directions on how to submit them.

  • Parents & Student Contract and Waiver

  • Student Authorization Form + Self-check out

  • Photo of Insurance Card

    • Front AND Back of insurance card needed*

  • Flight Itinerary Form

    • Required only if you will be needing Airport Transfer Service*

    • Upload of Flight Itinerary will also be needed

  • Ropes Course Waiver 

  • Field Trip Waiver 

  • Immunization Record Signed by a Healthcare Professional (Required)

  • Physical Exam Signed by a Healthcare Professional (Required)

    • Must have been completed within 3 years prior to program start date. Any medications listed on Physical must have a Medication on Program form.

  • Medication on Program 

    • Required if a student has prescription or over-the-counter medication during the program. Fill out one form per medication. 

      • *Note: SSB Staff are required to collect student medications upon check-in and students go to the Staff Office to access the medications. 

  • Individual Plan of Care for a Student With Special Health Care Needs or Disabilities

    • Required for students with any special needs (e.g. allergies, asthma, hearing/visual/physical impairments, chronic illness, developmental variations, panic attacks, etc.)
