How does it work and is it safe?

Here is what I believe to be the case. Manufactures have to make modern diesel engines run rich to lower combustion temperatures in order to reduce NoX emissions mandated by the government. By leaning up the air/fuel mixture you can actually run the engine much more efficiently, substantially increasing power, fuel economy and reducing carbon and soot deposits, however you will emit higher level of NoX particles into the environment. Keep in mind though that the NoX output is still going to be less than full-size diesel truck engines which are legal, they just have less stringent requirements to meet. This is what Volkswagen got in trouble doing on their diesel car engines, they used rich mode for vehicle testing, which lowered fuel mileage, power and NoX emissions but then reverted back to lean mode for regular driving, increasing fuel mileage, power and NoX emissions.
