Screening Tools

We would like to provide these three screenings tools to see if one might need to look more closely into motivating factors that contribute to using substances and problematic behavior. The completion and scoring of a screening tool allows one to be able to see what services might be appropriate for the one being assessed.

BELOW ARE THE THREE SCREENING TOOLS TO ASSESS THE SEVERITY LEVEL AN ADOLESCENT MIGHT HAVE WITH ALCOHOL, DRUGS OR MARIJUANA:
