Please find below the form for entering an act in a school assembly. Please be aware that you MUST have a teacher supervisor or sponsor to enter your act. If auditions need to occur prior to entering the piece, please complete these with the appropriate teacher (e.g. all dance pieces need to be approved by Ms. J.)

**Once your form has been submitted, it will be reviewed and you will receive a confirmation email that your entry has been received along with a form with choices for audition time. Every member of your group MUST attend the audition. Any member who does not attend will be cut.

All submissions must occur by the deadlines above. Otherwise, your submission(s) will not be included in your performance.
