Application for a Secondary Program at a School Outside the Resident Area
Use this form to:
- apply to a Specialized Program within the TDSB
- apply to an optional high school within the TDSB (not required for your child's home high school)
- fill in all areas
- Name of Requested Secondary School--the school to which you are applying (one form per school)
- Name of Sending School--the school your child currently attends
- Required Start Date--usually September of the following year, "September 2019"
- Grade--usually students apply for grade 9, but the form is also used for students applying for grades 10 to 12 as well
- Number of Credits--students applying for grade 9 usually have zero credits
- Surname is the same as Last Name
- Secondary Program Applications--students may apply to 2 Specialized Programs and 2 Regular Programs within the TDSB; write down where you are applying (TDSB only)
- Parent signature is mandatory to receive a signature from the school
- as the Guidance Counsellor, I am designated to sign and track these forms
- Click on TDSB High Schools (above under General Information) to find information about Closed/Limited schools (list available in January)
- Deadline for OA applications to secondary schools is Friday, February 1, 2019
