This form MUST be filled out COMPLETELY in order for it to be reviewed by your Counsellor. Students MUST attend ALL classes on their semester 2 timetable. It is the responsibility of the parents/guardians and the student to ensure that the courses being dropped are not PRE-REQUISITES for the student's post-secondary program. The deadline to submit course change requests is Friday, January 23rd.
