Our staff can assist you with all the necessary paperwork to obtain a birth or death certificate for events occurring in Switzerland County.Â
*We do not process birth/death certificates from other counties or states.
COST OF BIRTH CERTIFICATES
Birth Certificates are $12.00 each (per copy).
Death Certificates are $12.00 each (per copy).
We accept cash, check and credit card payments.
For out of county requests please visit the county of record or visit the Indiana Department of Health to order.
Who Can Obtain a Birth/Death Certificate
Attorney representing person named on record (must have I.D. for self and I.D./permission note from person named on record)
Brothers and Sisters
Caseworker from Division of Family and Children with court appointed guardianship papers
Children
Court Appointed Legal Guardian (must provide guardianship papers)
Grandparents
Individual named on the record (must be over age 18)
Law Enforcement personnel with I.D. and court order
Other county agencies left up to discretion of the registrar and the local health department
Parents of the person named on the record
Spouse
