Open Enrollment, New Hires, Retirees
HealthTrust and Benefit Related Forms: All the information you need related to the various benefits offered to Strafford Employees is listed below. All the forms you need for open enrollment, new hire, or change of benefits are listed below in electronic forms that can be downloaded to your computer, completed electronically, printed, signed/dated, and then submitted to the SAU for processing. Typing the documents results in docs that are more legible and less likely to experience kickback or mistakes. However, if you wish to print and complete by hand, that is acceptable. Please print clearly so as to avoid any processing issues.
For Open Enrollment, employees only need to complete the FSA form on an annual basis, whether using it or not. All other forms are to be used ONLY if you wish to make a change to your existing plan: ie. start or stop a benefit, add/remove dependents, update personal information, etc.

Here are the SBC's for the Medical Benefit Options (including prescription drug coverage) currently offered through HealthTrust. Distribution of these documents to employees is required by law during open enrollment.

These standardized letters from HealthTrust simply lets all of you know that we are holding open enrollment, some of the basic information needed to successfully navigate the process and info regarding child health insurance. CHIPRA distribution to all employees is required by law for open enrollment.

This section attempts to breakdown all the pay and benefits options available to each category of employee. If you do not fit perfectly within one of the categories below, please feel free to stop by the SAU to speak with HR and review your situation and help clarify any questions or concerns.
Current Employees & New Hire Forms
This section contains all of the electronic fillable forms you might need for open enrollment or for starting as a new employee. If you are an existing employee and you do not have any changes, DO NOT fill out Medical, Dental, LTD or Life. If you have a change, please complete the associated form. Everyone MUST complete an FSA form even if you DO NOT want FSA - just check appropriate box.
Once you complete any of the forms, please be sure to print, sign, date, and return to Rob Seaward in the SAU office. This can be via the main office, but DO NOT scan and email - too messy and need original signatures.
Retirees Section
Here are all of the important forms related to early retirement and ongoing retirement benefits through HealthTrust. If a retiree needs support or a form that is not listed, they will need to contact HealthTrust at 800-527-5001.
