I can ONLY administer OTC (over-the-counter) and Prescription Medication in the school setting with a fully completed "Medication Administration Form" (just click on the words!) or it can also be found under the Click & Print MEDICAL FORMSin the box to the left of this page. This form must be completed in its entirety which includes being signed by a licensed physician WITH the official office stamp to properly validate the order. A WRITTEN NOTE BY THE PARENT OR VERBAL PHONE CALL TO ADMINISTER ANY MEDICATION IS NOT ACCEPTABLE.
Registered Nurses work under the direction of a licensed physician NOT from a parent's request or a pharmacy bottle. This is the law under the New Jersey Nurse Practice Act.
You must provide your child's medication in a pharmacy container or the original unopened OTC box with your child's name clearly written on it. Several single dose pills or dose measured liquids are acceptable ONLY if in the original blister packet or plastic snap-spoon dose. Loose pills in an envelope are not acceptable. Please note the Health Office no longer contains its own supply of OTC medications. So you will need to provide your own bottle of liquid or blister pack of tablets.
If your child suffers from headaches, menstrual cramps, or gastric upset. I highly recommend you have a "Medication Administration Form" filled out by your physician so I can medicate them if necessary. This includes medication like Tylenol, Motrin, Advil, Tums, etc.
