The heart of this project is the Map My Path web platform — a one-stop, easy-to-use online space designed for students, families, and the professionals who support them. The platform makes it simpler to find information, plan ahead, and connect with the right services at the right time.
Personalized Planning Tools: Students and families can explore options for work, college, training, and independent living, and map out the steps to get there.
Local Resources: Search for programs, supports, and opportunities in your own community.
Training and Learning Modules: Access interactive training built for youth, families, schools, and providers, all designed to be accessible.
Progress Tracking: Keep plans, goals, and progress in one place, so everyone — students, families, schools, and partners — can stay on the same page.
Transition services can feel overwhelming, especially when supports are spread out across different agencies and organizations. The Map My Path platform brings everything together in a clear and connected way, making it easier for youth and families to see what’s possible and take the next step toward their goals.
The Map My Path platform isn’t just for the five-year grant. We’re building it to be sustainable and useful long after the project ends — a trusted resource for Colorado youth, families, and schools well into the future.
