One of the most powerful ways Map My Path is changing transition services is through School‐Based Interdisciplinary Teams. These teams bring everyone to the same table — the local school district, a School-Based Rehabilitation Counselor from Vocational Rehabilitation, and the local area’s Center for Independent Living.
The purpose is simple:
Coordinate services across agencies so students don’t get lost between systems
Create one clear student plan where schools, families, and community partners all work together toward the same goals.
By working as a team, we cut down on gaps, reduce duplication, and make sure students and families get the right support at the right time.
We are piloting this model in five communities across Colorado, each chosen to reflect different needs and experiences.
