NC Health Assessment

What is required?

A physician-completed North Carolina Health Assessment.

The exam date must be within one year of your child's enrollment.

If your child recently had a well-visit or annual physical (within the last calendar year) in another state, the provider who performed that exam may complete the NC form.

Who does this apply to?

All rising kindergarteners and students who have never attended a North Carolina Public School.

When is this due?

You have 30 days from the child's first date of enrollment to submit the NC Health Assessment.

The enrollment application can be processed without this documentation.

How do I submit?

Upload to your child’s enrollment application:

Student(s) > Student Document Uploads > Upload NC Health Assessment

Submit to the School Nurse or other designated school staff
