A copy of any court-ordered custody or guardianship documentation, or official DSS placement paperwork. A court order must have a judge's signature present.

Any student whose custodial parent or legal guardian has been designated by a court order.
This also includes students in the custody of DSS or those placed outside their home by DSS.

The legal/custody documentation or DSS placement paperwork must be submitted at the time of enrollment.
The application cannot be processed until this documentation is received.
