Student Accident Insurance 

LISD carries an accidental insurance policy that may be used for athletic injuries.  

The policy is a supplemental policy.  Payment coverage is based on the insurance company's schedule of benefits.  It may or may not pay a portion of the charges.

Additional Information::

• Staff Athletic Trainers must complete section I of the claim form

• initial treatment after an injury must occur within 60 days

• claim form must be submitted within 90 days of injury

• supplemental policy pays after primary insurance pays

• supplemental policy does not pay the remainder balance amount after primary insurance has paid
