To apply for admission to Southern Luzon State University (any campus), please complete the online application at the specified date.
Ensure you provide accurate personal information, academic history, details of extracurricular activities, and other requested admission-related information. Any false, intentional misrepresentation, and/or misdeclaration of actual information will be a ground for disqualification.
Submit all required documents, including your official academic records, certificate of income of both parent/s or guardians, certificate of residency, and other additional requirements.
Updates of your application will be communicated via email. Monitor your inbox regularly.

College Application
Requirements

Applicants shall prepare a clear scanned copy of the following documents before submitting the online college application for Southern Luzon State University (all campuses):

Maximum File Size: 2 MB

▪ Must have been taken within the last six months.
Dapat ay kinuha ang litrato sa loob ng nakaraan na anim na buwan.
▪ Must be clear, forward-facing the camera lens, and showing the full face, including the ears, if possible.
Makikita dapat ang buong mukha sa litrato kabilang na ang mga tainga.
▪ Only your face and shoulder should be included in the photo.
Tanging mukha at balikat lamang ang dapat kita sa litrato.
▪ Show natural skin tone.
Ipakita ang tunay na kulay ng balat.
▪ Eyeglasses and colored contact lenses should not be worn.
Bawal makitang suot-suot sa litrato ang salamin o contact lenses.
▪ Avoid thick or dramatic make-up. It must reflect your look on the day of the exam, such as with or without a beard.
Iwasan ang makapal na kolorete, dapat kung ano ang itsura mo sa litrato ay ito rin ang itsura mo sa araw ng
eksaminasyon tulad na lamang ng pagkakaroon ng bigote.
▪ Must be colored and on a white background
Dapat ay colored litrato at puti ang nasa likuran ng aplikante.
▪ The applicant's nametag (format: SURNAME, FIRST NAME, MIDDLE INITIAL) should be visible in the photo. This may be placed digitally or handheld while capturing the image.
Dapat ay kita sa ibabang bahagi ang pangalan ng aplikante (format: APELYIDO, UNANG PANGALAN,
PANGGITNA). Maaari itong in-edit o hawak-hawak habang kumukuha ng litrato.
▪ The digital photo must NOT be a scanned photo of any existing valid ID.

3. Clear Scanned Copy of Academic Records
For Grade 12 students expected to graduate at the end of the Academic Year:
Para sa mga Grade 12 student na magsisipagtapos ngayong academic year:
▪ Grade 11 Report Card duly
signed by the principal and
class adviser
For Senior High School Graduates:
▪ Grade 12 Report Card duly signed by the principal and class adviser
For Transferees and returnees:
▪ Transcript of Records (TOR)/ Copy of Grades. Applicants must not exceed two (2) semesters, and have a GWA of 2.25 with no failing grades and incomplete grades.
ALS Graduates:
▪ Certificate of Rating indicating “eligible for college admission”
4. Clear Scanned Copy of Certificate of Income for both parent/s or guardian/s or any of the following:
For 4Ps members:
▪ 4Ps Certification from MSWD/DSWD indicating the Household Number and name of the applicants as the beneficiary. Amount and source of income must also be indicated.

For Indigenous Peoples or Mga Katutubo:
▪ Certification from the National Commission on Indigenous People (NCIP)

For Enhancement of the Comprehensive Local Integration Program (E-CLIP) Beneficiary/ Payapa at Masaganang Pamayanan-Peaceful and Resilient Communities (PAMANA) Beneficiaries:
▪ Certification from DILG/ Office of the Presidential Adviser on the Peace Process(OPAPP)

For Parents employed in Government or Other Agencies:
▪ BIR Form 2316 (year 2024); or Certification from the Human Resource Office indicating its salary and other compensation-related details

For Solo Parent:
▪ Solo Parent ID, and Solo Parent Certificate from MSWD/DSWD with amount and source of income

For those not classified above (Unemployed, Housewife/ Househusband, no fixed income, guardianship, self-supporting student, and skilled workers):
▪ Notarized Sworn Statement issued by an attorney indicating the names of parents/guardians, and the amount and source of income

5. Certificate of Residency
▪ Certificate of Residency from the Barangay or Certification of Geographically Isolated and Disadvantage Areas (GIDA) from LGUs (if applicable)
6. If applicable, students who have become delegates or participants in any culture, art, and athletic competitions, must submit at least one of the following requirements:
▪ Letter or certification from the current school, signed by the principal & coach stating that the applicant was one of the official candidates (indicate to which specific sports or culture and art activities belonged and the highest level reached, example: Regional, National, International, etc.)

▪ Certification from the municipality or any institution (private or public) that the applicant was one of the official candidates (indicate to which specific sports or culture and art activities belonged and the highest level reached, example: Regional, National, International, etc.)

▪ Written endorsement from the Head of Sports and Cultural Program of your academic institution
