Documenting Contrast Extravasation or Contrast Reaction (must be done after EVERY extrav/reaction that you evaluate):

• Open an Epic progress note and use one of the following dot phrases in the body of the note:

• .radcontrastextravasation

• .radcontrastreaction

Orders to start IV through IV therapy clinic:

1. Open patient station, and select the infusion appointment (not the CT appointment). 

2. Open “Order Entry” 

3. Create a new order, and type in “Start IV” 

4. Select “IV line for CT scan (start)” 

5. Under comments type “20 gauge or larger, above the wrist” 

6. May make you select a diagnosis. You can add in primary problem, or if you don’t know you could type “abnormality on CT”, a generic indication. 

7. Sign order, may need to type in an attending in multiple fields.
