Course Request & Salary Increment Forms
These forms should be available in your school office.

THE TEACHERS' CENTER DOES NOT APPROVE THE COURSES. WE ONLY SEND OUT NOTIFICATIONS OF COURSES THAT THE DISTRICT WILL APPROVE
YOU MUST GET YOUR COURSES APPROVED BY THE DISTRICT!
Submit the first form when you start the courses
If you are starting a Master's Degree or Certificate Program, you must submit a listing of all the courses in the program for pre-approval at the start of your program with predicted start and end dates for each course. This is to prove that it is part of a Master's Degree program offered by a College or University and not something else.
In addition to that, you will need to submit the course approval form for each semester you take courses
MAXIMUM OF SIX CREDITS ALLOWED PER FALL AND SPRING SEMESTER. TWELVE CREDITS PER SUMMER SEMESTER. YOUR TRANSCRIPTS MUST INDICATE THE COURSE SEMESTER IT QUALIFIES FOR IN THE PROGRAM, BE IT A FALL, SPRING, OR SUMMER COURSE.
Submit the second form when you have completed enough credits.
6 credits allowed per Fall and Spring Semesters, 12 credits per Summer semester (summer begins the day after the last day of school in June).
Total of 18 credits allowed per year (Between start date July 1 to end date June 30)
