PE Credit:

I generally try to have the Date and eligible number hours per week to submit for PE Credit. This form needs your coaches signature or a member of the Parent Committee. Your student needs 80 hours, per semester, during the calendar School year. If you have questions regarding PE Credit, please contact the school directly.

You will need to download both the PE Waiver Request Form and Log sheet and submit these forms to the school for credit.
